Referring a patient:

Referring is easy and convenient. Simply phone me directly. The follow-up of a referral for mental health care is much higher when the physician “paves the way” so to speak by providing a linkage to the specialist. This may not always be needed. You can also print out our referral form to give to your patient. For those patients who may have difficulty making a call to a mental health professional, I have a range of enhanced referral practices which can help to ease the way. At the physician’s request – and with patient permission – I can then contact your patient directly to discuss the consult and arrange for their first meeting. Often, a gentle, personalized approach is appreciated and helps your patient feel very kindly cared for.
Collaboration:
The relationship your patient has with you and the care you provide is supported and reinforced by efficient communications giving you information you need about your patient’s mental health care. I am easily available and delighted to collaborate in any aspect of the care of your patient. At times, only minimal collaboration is needed, with other patients, more is desirable. At a minimum for all patients, following evaluation, I will provide you with my diagnosis and treatment plan and ask about your impressions and any concerns. You are subsequently provided with periodic updates of your patient’s progress. I will alert you of any critical incidents or changes in condition or treatment plan. Medication issues are commonly discussed. You will always be informed when treatment is ended and you are given a summary of results obtained and my recommendations for future care.
